Enriched living is a combination of housing, personalized supportive services and health care for individuals who may require assistance with activities of daily living such as bathing, dressing and medication management. It is designed to accommodate changing needs, while maximizing dignity, autonomy, privacy, independence, safety and choice. Staff is available 24 hours a day to meet both scheduled and unscheduled needs.
Residents maintain a level of independence while enjoying assistance, as needed, with Activities of Daily Living.
The staff of Clark Meadows includes a Program Manager, Lifestyle Coordinator, a Nurse/Care Coordinator, Case Manager and personal care aides who will help with daily needs. In addition, Clark Meadows has a partnership with UR Medicine Geriatrics Group, and the partnership includes on-site visits from an attending physician.
A single monthly fee covers all services provided by the community. This fee varies with apartment style and location. There is no entrance fee. Phone is a DOH requirement and therefore an additional charge. Internet in the apartments is extra as well.
No. Unlike some communities with multi-tiered pricing, we have adopted a flat rate structure that includes all services outlined in the contract.
Clark Meadows rates may be increased moderately in the future with a 30-day notice to residents. Any adjustment will be based on a cost-of-living increase as well as inflation.
We are a private pay community. Medicare does not cover Enriched Living. Check your Long Term Care Policy as it may provide a benefit for Enriched Living. Some Long Term Care Insurance policies will provide benefits.
Included in the monthly fee are:
 
  • Full apartment with a private bath and a kitchenette that includes a refrigerator and microwave oven.
  • Residents provide their own furnishings and personal items.
  • Assistance with activities of daily living – bathing, dressing, and medication assistance.
  • 24-Hour supervision - Clark Meadows always has personal care staff on duty.
  • Medication monitoring - Nursing staff monitors each resident’s medication regime and dosage guidelines, if needed.
  • Life Enrichment Programming - Clark Meadows’ Lifestyle Coordinator offers a range of programs to meet the desires and abilities of residents. Popular activities include poetry readings, current event discussions, music programs, crafts, and excursions to local restaurants and theaters.
  • Three home-cooked meals daily in an elegant restaurant setting. Each meal has a standard selection, with additional entrée choices.
  • Housekeeping - Clark Meadows provides daily and weekly housekeeping for residents.
  • Laundry Services – Our staff launders residents’ personal clothing, as well as bed linens.
  • Transportation Services - Clark Meadows coordinates with family and friends, or may provide local transportation to essential services (i.e. physician or dental visits) within a 12-mile radius. In addition, group transportation will be provided to certain recreational events.
  • Emergency call systems for each resident's apartment.
  • All utilities except telephone.
  • Other services are available at additional charge such as dry cleaning, additional personal care, personal shopping, and guest meals.
